Fees and Good Faith Estimate Statement
FINANCIAL POLICIES:
At 3B Wellness Counseling & Coaching, transparency about fees and billing is important. The following information is provided to help you understand how payment works for services.
Insurance Billing
The practice partner with Headway and Rula, which are platforms that help therapists accept insurance and manage billing.
If you choose to use insurance:
-
Your sessions will be billed through Headway or Rula, depending on the platform your insurance plan is connected with.
-
These companies verify your insurance benefits and submit claims on your behalf.
-
Your copay, coinsurance, or deductible will be determined by your specific insurance plan.
-
Payments for copays or session balances are typically processed through the platform connected to your care.
Because insurance plans vary, the exact amount you owe per session may differ depending on your policy. Benefits are checked with which platform your services are through and are reviewed with you via that platform prior to services begin, including no-show and less than 24 hour cancelations.
Self-Pay Fees
If you choose not to use insurance, or if your insurance plan is not in network, the self-pay fee for individual therapy sessions is $ 200.00 per 60-minute session.
Payment is due at the time of service unless other arrangements have been discussed. If No show or Less then 24 hour notice for cancelation, the full fee is owed.
Good Faith Estimate (No Surprises Act)
Under the No Surprises Act, clients who are not using insurance have the right to receive a Good Faith Estimate of the expected cost of services.
For example:
-
Individual therapy session ( 60 minutes): $200.00
-
Estimated cost for 10 sessions: $2,000.00
Please note that this estimate is based on the information known at the time and may change depending on the length or number of sessions.
You have the right to dispute a bill if it is at least $400 more than the Good Faith Estimate you received.
Questions About Billing
If you have questions about your benefits, copays, or billing through Headway or Rula, you can contact their support teams directly or discuss questions with me during our intake process.
If you are having questions about your self-pay, please be in contact with the practice.
My goal is to ensure that financial information is clear so you can focus on the therapeutic work.
Good Faith Estimate of Expected Charges
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
-
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
-
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
-
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
-
If you receive a GFE from us please make sure to save a copy or take a picture of it. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
